The Frankfurt Public Health Department confirmed that eight individuals have been diagnosed with malaria near the city’s main airport since July, and three of those patients have died. The majority of those affected are employees of Fraprot, the airport’s operating company.
Authorities attribute the cluster to “airport malaria,” a phenomenon in which a female Anopheles mosquito carrying the Plasmodium falciparum parasite is unintentionally transported on an aircraft or in a passenger’s luggage and then bites a person on arrival in a country where malaria is not endemic.
Achim Hörauf, director of the Institute for Medical Microbiology, Immunology and Parasitology at Bonn’s University Hospital, said in a press release that the situation is “a very rare occurrence, not a new frequent disease in Germany.” He explained that the parasite can survive briefly in the warmer parts of an airport before biting a host.
The current cases involve malaria tropica, the most dangerous form of the disease, which can cause severe complications such as lung fluid buildup, neurological damage, and acute kidney failure. If left untreated, about 20% of patients die, but timely diagnosis and targeted treatment can improve outcomes.
Symptoms of the infection are initially flu‑like – high fever, chills, headache, muscle pain, nausea, vomiting and diarrhea – making early recognition difficult, especially for people who have not traveled abroad. The health department urges anyone living or working near the airport who experiences these signs to seek immediate medical attention.
Rapid diagnostic tests on blood samples can detect the parasite, after which confirmatory laboratory analysis is performed. The Robert Koch Institute recommends that treatment be administered in hospitals with expertise in tropical medicine.
The last documented instance of airport malaria in Germany was recorded in 2023, according to the Epidemiologisches Bulletin of the Robert Koch Institute. With the onset of cooler autumn temperatures, especially at night, the environment becomes less favorable for mosquito survival, reducing the immediate risk of further transmission.
Nevertheless, experts warn that the incubation period of Plasmodium falciparum – seven to 15 days – means additional cases could still emerge even as the weather turns colder. Public health officials continue to monitor the situation and stress the importance of considering malaria as a possible diagnosis, even in the absence of recent travel.
